You will be required to bring the following to your initial appointment with Mr Humadi:

  • Your GP referral

  • Your medicare card

  • Details of you health insurance

  • Details of any Workcover or TAC claims

  • Any relevant medical records including imaging reports

  • Any questions you may have for Mr Humadi.

Fees & Payments

The cost of a consultation varies depending on the ‘Type of Appointment’ you require. Accounts are required to be settled on the day of your consultation. A proportion of the fee is rebatable from Medicare. We accept cash, cheque, EFTPOS, Visa and Mastercard. WorkSafe and TAC patients will only be seen if standard consultation fees are settled upfront on the day of consultation. Please note that this requires a co-payment.

Do I need a referral from my GP to see Mr Humadi?

Yes, all patients will need to be referred from their GP. This ensures you are eligible for Medicare rebates and that your primary healthcare provider can be included in your care. 

Does Mr Humadi see Workcover and TAC patients?

Yes, Mr Humadi sees Workcover and TAC patients, you will need an approval from your third-party insurer to see Mr Humadi. We recommed contacting your claims manager for guidance before your appointment to avoid any delays. Please bring your approval letter, claim details including claim number and date of injury and any other relevant 

What should I bring to my appointment?

Please bring your GP referral, medicare card, details of you health insurance, details of any Workcover or TAC claims, relevant medical records including imaging reports and any questions you may have for Mr Humadi.

Will I have out of pocket expenses associated with my appointment?

Yes, Mr Humadi's fee schedule can be discussed by calling the rooms. All fees must be paid on the day of service and Medicare rebates can be processed for you. 

Will my insurance cover the cost of my appointment?

As insurance policies typically only cover inpatient orthopaedic care, outpatient appointments are out of pocket costs. You will be eligible for a rebate through Medicare, please contact the rooms for more information.
